Allergy: No known drug allergy (NKDA).
Contraindications: BP check before each session; hold if SBP < 100 mmHg; fistula site inspected daily.
Pre-therapy assessment: Hb 10.4 g/dL; creatinine 4.2 mg/dL, K+ 4.9 mEq/L; fistula thrill normal; cardiology clearance on file.
Monitoring parameters
Intra-dialytic BP and pulse every 30 min; access-site inspection each session; interdialytic weight gain target < 3 kg; monthly Hb, K+, calcium-phosphate.
